The Coalition’s report only mentions the vicinity of the town of Tal Afar (تل عفر), for which the coordinates are: 36.375659, 42.451536. Due to limited satellite imagery and information available to Airwars, we were unable to verify the location further. Airwars assessment

A family of five including three children was killed in a US airstrike on the Albothiab Island area of Ramadi.

The deceased, the Kazem family, were later named by Al Fallujah News.

CENTCOM confirmed the casualty event on April 22nd 2016, noting: “On Dec. 12, 2015, in Ramadi, Iraq, during a strike on ISIL personnel at a suspected ISIL checkpoint, it is assessed that five civilians were killed after they unexpectedly moved into the target location after weapons were already in flight.“

It was also reported that the day before the incident, Iraqi government aircraft had targeted civilian taxis in the same area, killing two.

In July 2019 the Coalition provided Airwars with coordinates for this incident, accurate to within a 100 metre box.

Airwars assessment

Surviving family members told Buzzfeed investigators that five members of a family died in a suspected Coalition strike. The alliance later accepted responsibility.

Buzzfeed casenotes for the incident were later shared with Airwars:

“GPS Coordinates: 36°19’02.5″N, 43°06’35.1″E

Date and approximate time: March 3, at around 11am

Casualties: The nephew and four nieces of Zayd Younes Mohamed, all deceased. Four surviving children and brother, some with injuries.

Comments: This strike was not concurrent with the ISF assault into the neighborhood. ISIS had that day moved an anti-aircraft gun on the other side of a thin wall beside the home. The strike target the gun. It destroyed the home in the process; witnesses were not sure if the gun was also hit. Zayd and his brother buried the dead children. When they approached the Iraqi government for compensation, they were told that since the bodies were no longer at the site of the blast, the government could not prove that the dead were victims of the blast, and their case was rejected.”

In its August monthly casualty report the Coalition accepted responsibility for the event, noting that “March 3, 2017, near Mosul, Iraq, via media report: During a strike on an ISIS machine gun position engaging ISF, it was assessed that five civilians, not previously observed during the engagement, were unintentionally killed.”

Officials confirmed to Airwars that this was the al Mansour neighbourhood, and provided matching co-ordinates.

In an incident not previously tracked by Airwars, the Coalition confirmed the deaths of five civilians in an event in Mosul while survivors’ accounts add that at least three others were severely injured in the airstrikes.

Their June 2017 civilian casualty report noted “During a strike on an ISIS VBIED, it was assessed that five civilians were unintentionally killed.”

An investigation by the New York Times uncovered more details that “following orders from ISIS to evacuate their West Mosul neighborhood, two brothers, Majid Mahmoud Ahmed and Firas Mahmoud Ahmed, were driving with their families in two cars across town. At the same time, coalition forces were monitoring surveillance video of the area, looking to strike what intelligence had indicated was an up-armored vehicle carrying a car bomb. An official mistakenly identified the brothers’ cars as those carrying car bombs, and the strike was authorized. “I remember there was a big explosion, and I fainted,” said Abdul Hakeem Abdullah Hamash al-Aqeedi, pictured above. The cars were passing his house when the weapon hit. He lost an eye and had a plate put into his left leg. His son, Mustafa Hakeem Abdullah, had his left leg amputated from the thigh down. His nephew, who had been a nursing-school student, lost four toes on his left foot and one on his right foot and still has shrapnel in his leg. The brothers and their family members in the cars were all killed.”

The coordinates provided to Airwars – 36.324100, 43.127258 placed this event at Wadi Hajar neighbourhood in Mosul. There were two additional claimed events in Mosul that day – including an event at nearby Rifai which killed five civilians – although based on this location being 4.6 km distant, this does appear to be a separate incident.

Airwars assessment

Local and press sources said that four civilians were killed and twelve to sixteen others were injured, mostly children and women, when Coalition airstrikes targeted a residential building in the west of Mosul.

While Iraqi Spring spoke of “Coalition” airstrikes, the other sources blamed the US more specifically.

In two later posts, Iraqi Spring shared photos showing the huge destruction of buildings and several dead and wounded. It apologized for not including “for not showing all photos as they are shocking.” It spoke of US government aircraft.

The US-led alliance accepted responsibility for five deaths in the event in September 2018. Their monthly civilian casualty report noted: “During a strike on
ISIS fighting positions, VBIED facilities and an explosive factory, five civilians were unintentionally killed.”

Airwars assessment

Five civilians, including three women, were reported killed following airstrikes on Karabla. In particular, the Mahdi School was said to have been hit. Sources described heavy shelling by warplanes.

While the Yagein news agency referred to government aircraft as the perpetrators, it was not clear from other reports who was responsible for the strike, however.

In July 2019 the Coalition accepted responsibility for the deaths of all five civilians in this incident. Its monthly civilian casualty report noted: “Coalition aircraft conducted an airstrike against a Daesh weapons cache. Regrettably, five civilians were unintentionally killed due to their proximity to the strike.” The Coalition also provided Airwars with the location for this incident accurate to within a 100 metre box.

In May 2020, in its annual civilian harm report to Congress, the Pentagon conceded this as a US military incident.
